
I’m Connie, the founder and CEO of CSJ MDS Resource Group. I am a Registered Nurse with over 27 years of long-term care experience including Certified Nursing Assistant, Charge Nurse, MDS Coordinator, District Director of Care Management, Corporate Director of Care Management, VP of Utilization Management. I have a history of exceeding performance and financial goals while supporting multiple facilities in various states.
I earned an MBA in Health Care Administration, and am Advanced certified by AAPACN as a Resident Assessment Coordinator (RAC-CTA) as well as a QAPI Certified Professional (QCP). It is my mission to support Skilled Nursing Facilities’ Leadership and MDS teams in ethical MDS and reimbursement practices to improve clinical and financial outcomes.
With a collaborative approach involving Leadership, floor staff, MDS department and other key stakeholders, I am able to provide assessment, education and support for systems, processes and projects you need to improve clinical and financial outcomes.
The goal is to give you effective strategies and practical knowledge to ensure compliance with regulation, MDS coding, billing, and skilled clinical documentation.
